Understanding MedsCheck

MedsCheck is an Ontario government-funded program that allows eligible residents to receive a free, one-on-one medication review with a registered pharmacist. The program was introduced to improve medication safety, reduce preventable drug interactions, and support patients in managing complex medication regimens.

If you take three or more prescription medications for a chronic condition, you are likely eligible for a free Annual MedsCheck. Many patients who could benefit from this service are unaware it exists — or that it costs them nothing. At Longfields Pharmacy in Nepean, our pharmacists conduct MedsCheck reviews in a private, comfortable setting with ample time to address all your questions.

Types of MedsCheck

Annual MedsCheck
A comprehensive review of all your medications. Your pharmacist creates a personalized medication action plan.
Once per year
Who qualifies: Patients taking 3+ prescription medications for chronic conditions
MedsCheck Follow-Up
A shorter review to follow up on your medication action plan or address new concerns about your medications.
As needed
Who qualifies: Patients who have had an Annual MedsCheck and have questions or concerns
MedsCheck Adaptation
Conducted when a significant change is made to your medication regimen to ensure a smooth transition.
When medications change
Who qualifies: Patients whose medications have changed significantly
MedsCheck at Hospital
Helps patients understand medications prescribed after a hospital stay and identify any potential issues during the transition home.
Within 2 weeks of discharge

Who Qualifies for a MedsCheck?

To be eligible for an Annual MedsCheck, you must:

  • Be an Ontario resident with a valid OHIP card
  • Currently take three or more prescription medications for a chronic health condition
  • Not have had an Annual MedsCheck within the past 12 months

You do not need a referral from your doctor. Simply come in to Longfields Pharmacy or call us to schedule your review.

What Happens During a MedsCheck Review?

During your MedsCheck, your pharmacist will:

  • Review all your prescription and over-the-counter medications, vitamins, and supplements
  • Check for potential drug interactions or duplications
  • Confirm you understand why you are taking each medication and how to take it correctly
  • Assess whether your medications are working as intended
  • Discuss any side effects you may be experiencing
  • Answer all your questions about your medications
  • Create a personalized Medication Action Plan you can keep for reference
  • Share a summary with your family doctor (with your consent)

The review typically takes 20–30 minutes and is conducted in a private consultation area — not at the dispensing counter. You are encouraged to bring all your medication bottles, including vitamins and supplements you take regularly.

Is MedsCheck Covered by OHIP?

Yes. MedsCheck is fully funded by the Ontario government for eligible patients. There is no cost to you — the pharmacist is reimbursed directly by the Ministry of Health and Long-Term Care. You simply need a valid OHIP card.

If you have private drug insurance, MedsCheck is still billed to OHIP and does not affect your private insurance coverage.

Do I need a referral from my doctor to get a MedsCheck?

No referral is needed. Simply contact Longfields Pharmacy directly to schedule your MedsCheck.

Can I get a MedsCheck if I do not have a family doctor?

Yes. MedsCheck does not require a family doctor. If you take three or more prescription medications for a chronic condition and have an OHIP card, you are eligible.

What should I bring to my MedsCheck appointment?

Bring all your medication bottles — prescriptions, over-the-counter medications, vitamins, and supplements. Also bring your OHIP card.

Will my doctor be informed about my MedsCheck?

With your consent, your pharmacist can send a summary of your Medication Action Plan to your family doctor or specialist. This is optional.
