It is more comprehensive than a standard MedsCheck and specifically addresses issues common in older adults, such as medications that may increase fall risk, cognitive side effects, inappropriate prescribing in the elderly (the Beers Criteria), and the cumulative burden of polypharmacy. The pharmacist provides a written medication management plan and can communicate recommendations to the prescribing physician.
